Clinical Study of Acute Renal Failure

Abstract
Clinical study of acute renal fail re for 191 patients who had been hospitalized at Dept. of Int. Med. SNUH from Nov. 1971 to Aug. 1976 were presented and pertinent observation were as
followings.
1) Three patient developed acute renal failureafter surgery and trauma, 10 patients after delivery or abortion and 178 patients medically of which 123 patients were by Korean hemorrhagic fever.
2) Average lengths of oliguric stage were 8.8 days; early diuretic stage, 5.8 days; late diuretic stage, 11. 5 days; the recovery stage, undetermined.
3) The highest serum BUN in each patient ranged from 55.4 to 300mg%. Hyperkalemia appeared in 48.8% of the patients who were not dialyzed initially. Hyperbilirubinemia vas evident in 26% of patients.
4) Albuminuria was present in almost all patients, glycosuria in 29%, hematuria in 63% and pyuria in 43% of patients.
5) Infection contributing to the cause of acute renal failure was observed in 17.6% of the patients and 52% of all patients had respiratory or urinary tract infection as complication.
6) The peritoneal dialysis or hemodialysis was done in 25% of patients and induced clinical improvement in all except 2 patients.
7) Histopathological study was performed in 18 patients, of which 16 patients showed picture of acute tubular necrosis one patient of acute interstitial nephritis and another of bilateral cortical necrosis.
8) The mortality rate was 34%. The presumptive cause of death were hyperkalemia, shock and pulmonary edema in order of frequency.
9) Non oliguric acute renal failure was observed in 6 patients(8.8%)
